Understand the model

  1. Definition: a versioned question and its evidence and assessment criteria.
  2. Question instance: that question applied to a particular subject, time, and parameters.
  3. Record: attributed evidence references, observations, hypotheses, assessments, decisions, and actions.
  4. Revision: a new entry preserving the old one and identifying dependent decisions that need review.

An assessment can be applicable, not applicable, or unresolved. Only an applicable assessment has a yes, no, or unknown outcome. Missing evidence does not mean “no.” A recorded action does not grant permission to execute it.
